加味白茅根汤治疗慢性肾炎疗效观察

笔者系用白茅根汤(1)治疗慢性肾炎疗效显著,多年将
此方用于临床,每试易效,现报道如下。
1 一般资料
笔者共收治慢性肾炎患者 26例,男 15例,女 11例,
年龄 20~58岁,病程均在三年之上,最长达 22年。
2 方药组成
白茅根汤 50g,薏苡仁 20g,赤小豆 20g,党参 30g,黄
芪 20g。
3 煎煮方法
将原煎方法稍加改进,将药加水量超过高容器内药面二
横指,以久火加热至沸腾 10min,将药液滤出;再加水至距
药面下二横指煎煮 10min滤出;仍加水至药面下二横指煎煮
10min后滤出,将三次药液混匀,分早晚二次服用,一日一
剂。
4 治疗结果
26例患者均为慢性肾炎,经上方治疗 24例,临床症状
完全消失,尿常规检查连续三次均正常,治愈率 92.3%,2
例显效,但病情已得到控制。
5 典型病例
例 1:女,49岁,工人。1992年 3月来诊时,自诉慢
性肾炎已达 3年之久,反复发作均未能治愈,症见:面目四
肢水肿,腰痛面红目赤口渴,舌质红舌苔黄,尿少色赤,脉
细无力,尿常规异常。笔者嘱其连服加味白茅根汤 62剂后,
临床症状全部消失,尿检恢复正常值,体力充沛,随访至今
全未复发。
6 讨论
笔者认为,慢性肾炎多为感受湿热之邪而致病,病人反
复发作,湿热久留,耗伤气阴;肾脏渐虚,则转为慢性,本
方白茅根味甘性寒,清热利水,凉血止血;薏苡仁味甘微寒,
清热利湿;赤小豆甘酸,性平,清湿热,利尿消肿,三味要
药配伍,清利湿热,滋养阴液。又因久病必虚,辅以党参、
黄芪,使气虚证候得以明显改善,从而达到了益肾利湿,补
气活血的调治原则。

Determination of plasma endothelin-1 and nitric oxide
in children with simple nephrotic syndrome

Abstract Objective: To study the change patterns of plasma nitric oxide (NO) and endothelin-1 (ET) in children with
nephrotic syndrome. Methods : The plasma NO and ET concentrations were determined in 27 children with nephrotic syndrome, and
20 healthy children as the control group by spectrophotometry and RIA. Results: 1.NO and NOS levels in patients with nephrotic
syndrome (44.90±9.74μmol/L,13.84±3.44U/mL)were significantly higher as compared with those in the control group (80.46±
13.43μmol/L,37.20±3.80U/mL).2.ET level in patients with nephrotic syndrome (194.41±46.42pg/ml)were significantly lower as
compared with that in the control group(79.02± 13.37pg/mL).3.Plasma NO was positively correlated with the plasma
NOS(r=0.8798,P<0.01) and negatively correlated with the plasma ET(r=-0.6965, P<0.01). Conclusion: NO, NOS and ET play an
important role in the nephrotice syndrome. NO levels in children with nephrotic syndrome had negatively relationship with ET,
suggesting that NO and ET both might take part in the pathophysiology in nephrotic syndrome.
